Packet and payment must be received by Thursday, March 20, 2025 in order to reserve your seat.

A B&W or color copy of a valid picture ID (examples: passport, copy of the front of a driver’s license)

A completed Checklist for Lawful Presence

A completed Citizenship Affidavits that has been notarized. Please complete the correct affidavit (only one needed); we can notarize if needed. If you require our notary services please arrive 15-20 minutes before the start of the class.

Payment of $175 for each attendee. Payment for the course is due before the test date.
